Clinics run by our sponsors assess both physical and mental fall risk, but typically meet with patients only once per month. This limits their ability to track progress, monitor exercise adherence, and adjust treatments. AI-PEER is a mobile health application designed to extend PEER care into the home, supporting older adults with sedentary lifestyles, reduced mobility, or high fall risk. The system is based on the PEER framework, which aligns a patient’s perceived fear of falling with their actual physical balance. AI-PEER collects physical balance data and uses surveys to assess perceived fall risk. The app includes pose-based exercise evaluation, motivational feedback, and fall-risk scoring, supporting both physical improvement and user engagement. It integrates exercise tracking, AI feedback, and secure data sharing to enable continuous monitoring outside clinical visits. This allows clinicians to better assess patient adherence and progress over time. Overall, AI-PEER provides an accessible, scalable, and low-cost solution to improve fall-risk management and support safe physical activity in older adults.
